VAWAS Advanced Nurse Practitioner

We are recruiting an enthusiastic motivated experienced Advanced Nurse Practitioner for our VAWAS TEAM (Vulnerable Adults Wrap Around Service).

The successful candidate will be an exceptional team player with good communication and interpersonal skills who is enthusiastic and committed to join the team on a permanent basis. We are looking for a dedicated, enthusiastic practitioner who share our values and our passion for good quality patient focused care to our most vulnerable patients.

The ideal candidate will have a sound background in chronic disease management, first contact and telephone triage, assessing treating and reviewing all ages and conditions.

The candidate must have at least two years' experience as an Advanced Nurse Practitioner and hold a non-medical prescribing qualification.

The role requires travel to and from patient homes and the GP Practice aligned care homes, therefore the successful candidate must have means of transport and drive, mileage will be reimbursed at the company rate.

The successful applicant for this post will be joining us at a particularly exciting time as we develop and expand our services.

As Clinical Practitioner you would have the freedom and authority to act and accept accountability for your actions. This level of practice is characterised by autonomous decision making for complex multi-dimensional problems using high level knowledge and skills.

The successful candidate would have the benefit of support from the wider multidisciplinary team within the practice and would attend regular practice clinical & educational meetings. The multidisciplinary team are well integrated, demonstrating a strong team spirit and enthusiastic approach to practice development.


Key Duties

  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for registered patients presenting with an undifferentiated diagnosis and long term chronic conditions.
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
  • Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs
  • Deliver LTC reviews and venepuncture when required
  • Complete DNACPR forms
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
  • Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan

Key Skills, Qualifications and Client Requirements

  • Relevant nursing/health MSc / Degree in Advanced Nursing Practice.
  • Minimum 3 years post registration experience
  • At least 2 years recent primary and/or community nursing experience
  • Nurse-led management of minor illness, minor ailments
  • Nurse-led triage experience.
